RESUMO

Metastasis remains the most common cause of death in cancer patients. Inhibition of metalloproteinases (MMPs) is an interesting approach to cancer therapy because of their role in the degradation of extracellular matrix (ECM), cell-cell, and cell-ECM interactions, modulating key events in cell migration and invasion. Herein, we show the cytotoxic and antimetastatic effects of the third fraction (FR3) from Bauhinia variegata candida (Bvc) stem on human cervical tumor cells (HeLa) and human peripheral blood mononuclear cells (PBMCs). FR3 inhibited MMP-2 and MMP-9 activity, indicated by zymogram. This fraction was cytotoxic to HeLa cells and noncytotoxic to PBMCs and decreased HeLa cell migration and invasion. FR3 is believed to stimulate extrinsic apoptosis together with necroptosis, assessed by western blotting. FR3 inhibited MMP-2 activity in the HeLa supernatant, differently from the control. The atomic mass spectrometry (ESI-MS) characterization suggested the presence of glucopyranosides, D-pinitol, fatty acids, and phenolic acid. These findings provide insight suggesting that FR3 contains components with potential tumor-selective cytotoxic action in addition to the action on the migration of tumor cells, which may be due to inhibition of MMPs.

This study evaluated the effect of the core substrate type (dentin and composite resin) on the retention of crowns made of yttrium oxide stabilized tetragonal zirconia polycrystal (Y-TZP), submitted to three inner surface conditionings. For this purpose, 72 freshly extracted molars were embedded in acrylic resin, perpendicular to the long axis, and prepared for full crowns: 36 specimens had crown preparations in dentin; the remaining 36 teeth had the crowns removed, and crown preparations were reconstructed with composite resin plus fiber posts with dimensions identical to the prepared dentin. The preparations were impressed using addition silicone, and 72 Y-TZP copings for the tensile test were produced. Cementation was performed with a dual-cured cement containing phosphate monomers. For cementation, the crown preparation (dentin or resin) was conditioned with the adhesive system, and the ceramic was subjected to one of three surface treatments: isopropyl alcohol, tribochemical silica coating, or thin low-fusing glassy porcelain layer application plus silanization. After 24 hours, all specimens were submitted to thermocycling (6000 cycles) and placed in a special tensile testing device in a universal testing machine to determine failure loads. The failure modes of all samples were analyzed under a stereomicroscope. Two-way analysis of variance showed that the surface treatment and substrate type (α=0.05) affected the tensile retention results. The dentin substrate presented the highest tensile retention values, regardless of the surface treatment. When the substrate was resin, the tribochemical silica coating and low-fusing glaze application plus silanization groups showed the higher retention values.

Problem: The use of central venous catheters in intensive care units has been identified as an important risk factor for infection in the bloodstream. Objective: To examine the evidence in the literature about the indications and uses of central venous catheter insertion, the risk of infection, the professional conduct in relation to safe practices related to this type of device. Methods: Narrative review, which assesses the risk of infection in central venous catheters in intensive care units; by bibliographic study in database of BVS, in the time gap from 2005 to 2010 using the keywords: infection, intensive care unit , central venous catheterization and nursing care. Results: The analysis identified 03 thematic units: 1. Complications associated with central venous catheter, which evaluates the evidence from a framework of systemic infection where vascular access is implicated as a possible source, and lists the factors that influence the onset of complications associated; 2. Adoption of the correct technique of catheter-related coverage, which recommends to use the coverage appropriate to prevent skin toxicity, and increases patient comfort and satisfaction, besides attention to time of the use and signs of inflammation, 3. Realization of safe practices for the maintenance of the catheter, involving the manipulation control and use of protocols. Conclusion: We conclude that the quality of care for patients with this device is directly related to controlling the risks of infection; where efforts, not only the nurse, but the health team will be enable the application of evidence to guide a safe clinical practice and contribute to improving the quality of health care and nursing.


Problema: A utilização de Cateteres Venosos Central em Unidades de terapia intensiva tem sido apontada como importante fator de risco para infecção na corrente sanguínea. Objetivo: examinar as evidências encontradas na literatura sobre as indicações e formas de utilização do Cateter venoso central, sua inserção, o risco de infecção, as condutas profissionais em relação as práticas seguras relacionados a este tipo de dispositivo. Metodologia: Revisão narrativa, que avalia o risco de infecção em Cateteres Venosos Central em Unidades de terapia intensiva; de cunho bibliográfico realizado nas bases de dados da BVS, no hiato temporal de 2005 a 2010, utilizando os descritores: infecção, centro de terapia intensiva, cateterismo venoso central e cuidados de enfermagem. Resultados: A análise dos resultados permitiu identificar 03 unidades temáticas: 1. Complicações inerentes ao cateter venoso central; que avalia as evidências a partir de um quadro de infecção sistêmica no qual o acesso vascular é implicado como possível fonte; relaciona os fatores que influenciam no aparecimento de complicações associadas; 2. Adoção da técnica correta relacionada a cobertura do cateter, que recomenda a utilização de cobertura e trocas adequadas para prevenir a toxicidade cutânea, e aumenta a satisfação e o conforto do paciente; além de estar atento ao tempo de utilização e sinais flogísticos; 3. Realização de práticas seguras para a manutenção do cateter, onde identificamos que as práticas seguras para a manutenção do Cateter envolvem o controle na manipulação e o uso de protocolos. Conclusão: Conclui que a qualidade da assistência à pacientes com este dispositivo está diretamente relacionada ao controle dos riscos de infecção; onde esforços, não somente da enfermeira, mas de toda a equipe de saúde envolvida a fim de viabilizar a aplicação das evidências para nortear uma prática clínica segura que contribua para a melhoria da qualidade da assistência à saúde e de enfermagem.

B3LYP/6-311++G(d,p) calculations were used to predict some molecular properties of the C2H6N+...BeH2, C2H6N...MgH2, C3H8N...BeH2 and C3H8N+...MgH2 dihydrogen-bonded complexes. In these systems, it was demonstrated that the C2H6N+ and C3H8N+ protonated rings are potential candidates to bind with protonic hydrogens derived from alkaline earth metal compounds, BeH2 and MgH2. In terms of structural parameters and quantification of the dihydrogen bond energies, we should mention that the C2H6N+ three-membered ring provides the formation of stronger bound systems, which are 4.0 kJ mol-1 more stables than C3H8N+ four-membered ones. As complement, the analysis of the infrared spectrum indicated that red-shifts and blue-shifts are occurring in the N-H bonds of both C2H6N+ and C3H8N+ cationic rings. However, these two vibrational shifts were also verified on BeH2 and MgH2, what lead us to affirm that cationic compounds derived from small nitrogen rings and earth alkaline molecules are able to form unusual dihydrogen-bonded complexes by means of distinct spectroscopic phenomena, the red-shits and blue-shifts.

INTRODUÇÃO: Os crescentes avanços tecnológicos desenvolvidos para o tratamento de distúrbios de condução cardíaca vêm proporcionando aos pacientes melhores condições de vida. As escalas de classificação funcional e questionários de qualidade de vida (QV) constituem uma forma suplementar de avaliação dos aspectos físicos, emocionais e funcionais dos pacientes. Entretanto, permanece a seguinte questão: existe correlação entre a classe funcional (CF) e a percepção da QV em usuários de marcapasso (MP)? OBJETIVO: O objetivo do estudo foi avaliar se existe correlação entre classe funcional (CF) e QV em portadores de MP cardíaco definitivo. MÉTODOS: Foram avaliados 14 usuários de MP. Para avaliar CF, foi utilizada a escala de atividade específica proposta por Goldman, e, com objetivo de avaliação da QV, foi aplicado o questionário Aquarel associado ao SF-36. Com o objetivo de verificar se existe correlação entre as variáveis, foi aplicado o teste de correlação de Spearman, considerando como significativo a< 0,05. Para a análise dos dados, foi utilizado Software SPSS for Windows versão 10.0. RESULTADOS: A CF correlacionou-se inversa e significativamente com a QV avaliada pelo Aquarel em seus três domínios: desconforto no peito (r= -0,666; p= 0,009); dispnéia (r= -0,604; p= 0,022) e arritmia (r= -0,550; p= 0,041). Já em relação ao SF-36, dos seus oito domínios, três estabeleceram uma correlação significativa com a CF: capacidade funcional (r= -0,745; p= 0,002); dor (r= -0,667; p= 0,009) e vitalidade (r= -0,591; p= 0,026). CONCLUSÃO: No presente estudo, encontrou-se correlação significativa entre CF e QV, sugerindo que as escalas de classificação funcional podem refletir aspectos da QV de portadores de MP.


INTRODUCTION: Growing technological progress in treating patients with heart conduction disturbances has provided such patients with better life conditions. Functional classification (FC) scales and quality of life (QOL) questionnaires are additional means for evaluating patients' physical, emotional and functional characteristics. However, the question remains as to whether there is any association between FC and perception of QOL among pacemaker users. OBJECTIVE: To evaluate whether there is any correlation between FC and QOL among definitive cardiac pacemaker users. METHOD: Fourteen pacemaker users were evaluated. To assess FC, the specific activity scale proposed by Goldman was used. To evaluate QOL, the Aquarel questionnaire was used in association with SF-36. The Spearman correlation test was applied to investigate whether there was any association between the variables, considering p< 0.05 to be significant. The SPSS for Windows software, version 10.0, was used for the data analysis. RESULTS: There was a significant negative correlation between FC and QOL through evaluation by Aquarel questionnaire in its three domains: chest discomfort (r= -0.666; p= 0.009); dyspnea (r= -0.604; p= 0.022); and arrhythmia (r= -0.550; p= 0.041). Among the eight domains of SF-36, three showed a significant correlation with FC: physical functioning (r= -0.745; p= 0.002); pain (r= -0.667; p= 0.009); and vitality (r= -0.591; p= 0.026). CONCLUSION: In the present study, a significant correlation was found between FC and QOL, thus suggesting that functional classification scales may reflect aspects of QOL among pacemaker users.

B3LYP/6-311+G(d,p) calculations were used to explore the geometry, intermolecular energy and the vibrational harmonic spectrum of heterocyclic complexes formed between 2,5-dihydrofuran and thiophene cyclic ethers and the HCl and HF acids. The simulated structures of these hydrogen complexes are discussed in terms of the linearity deviation of the n...HX hydrogen bond. Theoretical results are satisfactory as compared to the experimental equilibrium structure. The energies of the hydrogen bonds were determinate through the difference between the complex and its correspondent isolated monomers. Moreover, to obtain the correct energies of the hydrogen bonds, it was included the values of the zero point vibrational energy and the basis set superposition error. The infrared spectra reveal the direct relationship between the distance of the hydrogen bond and its stretching frequencies, as well as a good interpretation of the bathochromic effect of the HCl and HF stretching modes from intermolecular charge transfer.

The objective of the study was to assess knowledge and attitudes about human papillomavirus (HPV), cervical cancer, and Papanicolaou (Pap) smears among young women. A questionnaire was administered to 204 women aged 16-23 years, attending a public clinic. Data were gathered on sociodemographic characteristics, knowledge, and attitudes related to HPV. Overall, 92% of women reported current/previous sexual activity, 42% perceived themselves at high risk of acquiring a sexually transmitted disease, 67% did not know that HPV can cause cervical cancer/warts, and only 10% acknowledged that HPV might lead to cervical cancer. In general, women had a poor knowledge on HPV diagnosis/treatment, condyloma signs, and Pap smear test. The main reasons for not having a Pap smear test done before were embarrassment (63%) and fear of pain (61%). Knowledge of HPV infection and cervical cancer was low in this urban young population. Our findings recommend for greater HPV education of the public and health care practitioners.

The optimised geometries of heterocyclic hydrogen-bonded complexes, C2H4O...HX and C3H6O...HX, where X=F or Cl, were determined at DFT/B3LYP/6-311++G(d,p) computational level. Structural, electronic and vibrational properties of these complexes are used in order to compare the strained ring, which confer the great reactivity of these heterocyclic rings with monoprotic acids, forming a primary hydrogen bond. A secondary hydrogen bond between the hydrogen atoms of the CH2 groups and the halide species also takes place, thus causing a nonlinearity (characterized by the theta angle), in the primary hydrogen bond.

Ab initio calculations are performed at the MP2/6-311++G(d,p) and DFT/B3LYP/6-311++G(d,p) theoretical levels to obtain geometries, H-bond energies and harmonic infrared vibrational properties for the Cs symmetry structures of heterocyclic hydrogen-bonded complexes, CnHmY-HX. The H-bond lengths in DFT/B3LYP calculation level are in better agreement with the experimental values than the MP2 results. The geometry optimization are interpreted in terms of hydrogen bond nonlinearity represented by theta; and phi angles, once the hydrogen bond is formed among n-electrons pairs of the heteroatom in heterocyclic and the hydrogen atom in HX. The hydrogen bond energy after of the zero-point vibrational energy (ZPE) and basis set superposition error (BSSE) corrections are overestimated at DFT/B3LYP, whereas the MP2 BSSE corrections are very large than corresponding DFT/B3LYP. For example, the BSSE corrections for the C2H4S-HNC complex are 7.60 and 0.09 kJ mol(-1) in MP2 and DFT/B3LYP calculations levels, respectively. The new vibrational modes in infrared harmonic spectrum arising from complexation show several interesting features, especially the intermolecular stretching mode.

We present a theoretical study through MP2 ab initio molecular orbital calculations and B3LYP density functional theory with the 6-311++G(d,p) basis set of the heterocyclic hydrogen complexes, CnHmY-HX, where CnHmY = C2H4O, C2H5N and C2H4S, and X=F or Cl. This study aided in the elucidation the main changes in the structural, electronic and the vibrational properties in isolated species, due the hydrogen complexes formation, CnHmY-HX, revealing systematic tendencies in these chemical systems studied. The complexes has CS symmetry, with the HX subunit lying in the plane perpendicular to that of CYC nuclei of heterocyclic and acting as proton donor in forming a hydrogen bond to the heteroatom, Y. A weak secondary interaction between the CH2 groups of heterocyclics and the X atoms in HX causes a significant nonlinearity of the primary hydrogen bond. The hydrogen bond linearity deviations in these complexes due to secondary interactions are represented by theta angle. The MP2 intermolecular distances of complexes C2H5N-HF, C2H4O-HF and C2H4S-HF correspond the 1.652, 1.671 and 2.164 A, respectively, these results are in excellent agreement with experimental results of 1.700 and 2.193 A found for the last two complexes. In the same way, the MP2 values to theta angle, 14.7, 19.1 and 16.8 degrees, has a better reproduction in the experimental results of 16.5, 21.0 and 16.8 degrees, get to the C2H4O-HCl, C2H4S-HCl and C2H4S-HF complexes, respectively.

OBJECTIVE: This study was performed to observe the number of pacemakers that had never been reprogrammed after implantation, and the effect of optimised output programming on estimated longevity of pulse generators in patients with pacemaker METHODS: Sixty patients with Teletronics Reflex pacemakers were evaluated in a pacemaker clinic, from the time of the beginning of its activities, in June 1998, until March 1999. Telemetry was performed during the first clinic visit, and we observed how many pulse generators retained nominal output settings of the manufactures indicating the absence of reprogramming until that date. After evaluation of the capture threshold, reprogramming of pacemakers was performed with a safety margin of 2 to 2.5:1, and we compared the estimated longevity based on battery current at the manufacturer's settings with that based on settings achieved after reprogramming. RESULTS: In 95% of the cases, the original programmed setting was never reprogrammed before the patients attended the pacemaker clinic. Reprogramming the pacemaker prolonged estimated pulse generator life by 19.7+/-15.6 months (35.5%). CONCLUSION: The majority of the pacemakers evaluated had never been reprogrammed. Estimated pulse generator longevity can be prolonged significantly, using this simple, safe, efficacious, and cost-effective procedure.
